Before you are enrolled in ACT, Care Support Specialists will:

  • Answer your questions by phone
  • Explain to you how the ACT Support Program works
  • Help you get enrolled in the program
  • Connect you with your dedicated ACT Care Coordinator at the appropriate time

  • Someone who will be there for you throughout your treatment journey
  • Available by phone or email as you need them (translation services available for non-English-speaking people)

  • Dedicated to providing the highest levels of customer service

The Amylyx Care Team is relentless in helping to break down barriers for people living with ALS and their loved ones, partnering to provide simple solutions so they can focus on what matters most.

ACT Care Coordinators—your gateway to a wide range of support

Through your ACT Care Coordinator, you will be connected with the following support and the appropriate ACT team members who help in each of these areas.

In addition to enrolling you into the ACT Support Program, we can:

  • Help you understand your insurance coverage
  • Provide support to your doctor’s office with paperwork they may need to fill out to help you gain access to treatment

Our ACT Care Educators are registered nurses who can provide additional education about ALS and your treatment.*

We can connect you with other groups that support people living with ALS, including local or regional community advocacy groups in your area.

We partner with specialty pharmacies to help:

  • You receive your medication on time
  • Ensure you know how to take your medication
  • Provide you with additional medication counseling, as needed, from a trained pharmacist working in collaboration with your doctor, who is always your best source of treatment information
  • Coordinate delivery of medication to your home

Financial Assistance through ACT Support Program Initiatives

We offer comprehensive financial assistance for eligible individuals

We strive to find affordable solutions for you through the following programs:

$0 Co-Pay Program*

If you are commercially insured (meaning you have private insurance, not Medicare, Medicaid, or any other state or federally funded insurance program), you may be eligible for our co-pay assistance program. Your ACT Care Coordinator can help you see if you qualify for a $0 co-pay.

Interim Access Program

We can assess your eligibility to receive up to 2 months of RELYVRIO while you’re waiting for your insurance company to approve your treatment. This program is available on a case-by-case basis if you’ve been prescribed RELYVRIO for the first time and are commercially insured.

Patient Assistance Program

If you do not have insurance or do not have insurance that covers your treatment, the ACT Patient Assistance Program may be able to assist you. Your ACT Care Coordinator can work with you to determine eligibility.

*Out-of-pocket costs related to medication, appointments, evaluations, testing, or other related services are not covered by the RELYVRIO Co-Pay Program. The RELYVRIO Co-Pay Program is not available for prescriptions purchased under Medicare, Medicaid, TRICARE, or other federal- and state-funded programs. Amylyx reserves the right to amend or terminate the Program at any time without notice. Co-pay amounts after applying co-pay assistance may depend on the individual's insurance plan and may vary. The RELYVRIO Co-Pay Program is intended to help individuals afford RELYVRIO.

How do you enroll in ACT?

When you and your doctor decide that RELYVRIO is right for you, it is important to work together to enroll in ACT. Together, you can fill out the RELYVRIO Enrollment Form, which will give you access to support from the ACT team and serves as your prescription for RELYVRIO.

IMPORTANT SAFETY INFORMATION

Before you receive RELYVRIO, tell your doctor about all of your medical conditions, including if you:

  • Have pancreas, liver, or intestinal problems
  • Have heart failure, including congestive heart failure
  • Have high blood pressure
  • Have kidney problems
  • Are pregnant or plan to become pregnant. It is not known if RELYVRIO will harm your unborn baby
  • Are breastfeeding or plan to breastfeed. It is not known if RELYVRIO passes into your breast milk. You and your doctor should decide the best way to feed your baby

Tell your doctor about all the medicines you take, including prescription and over-the-counter medicines, vitamins, and herbal supplements and any taurursodiol products, such as tauroursodeoxycholic acid (TUDCA).

RELYVRIO may affect the way other medicines work, and other medicines may affect how RELYVRIO works.

What are the possible side effects of RELYVRIO?
RELYVRIO may cause serious side effects, including:

  • Changes in bile acid levels. RELYVRIO may increase bile acid levels and cause worsening diarrhea if you already have problems with your liver, bile ducts, or pancreas. Your doctor should monitor you for these side effects. Some disorders of the pancreas, bile ducts, or intestines may also make it harder to absorb RELYVRIO
  • Salt (sodium) retention. RELYVRIO contains a high amount of salt. For people who are sensitive to salt intake, such as people with heart failure, high blood pressure, or kidney problems, limit the amount of salt you eat and drink. Talk to your doctor about the total amount of daily salt that is right for you. Your doctor will monitor you for signs and symptoms of salt retention during your treatment with RELYVRIO

The most common side effects of RELYVRIO include diarrhea, abdominal pain, nausea, and upper respiratory tract infection.

Tell your doctor if you have any side effect that bothers you or that does not go away. These are not all the possible side effects of RELYVRIO. Call your doctor for medical advice about side effects.
